Design your own Forms and Questionnaires!

Tracker’s T-Forms Add-on module allows you to dynamically create your own custom Forms and Questionnaires. Forms can be created with a different variety of user-definable question types to make data entry easy, accurate, and consistent.

Once you design a custom Form or Questionnaire, it dynamically appears in Tracker's Contact Screen allowing you to enter specific information for each patient using the Forms you have created.

The patient-specific information entered in each Form can then be printed or automatically be placed in a Microsoft Word document using T-Word's mail merge facility. Therefore, you can easily create sophisticated and personalized documents including custom letters, lab forms, and various questionnaires!
